Where should the phone number and quote form go on a contractor website?

Put a tappable phone number in the top right of the header on every page, repeat it in the footer, and place a short form above the fold on the home page and every service page. Add a second call to action at the bottom of each page, because visitors who read that far are the ones ready to act.

On mobile, a sticky bottom bar with Call and Get a Quote buttons is usually the single highest-impact change on a contractor site. Do not bury the number on a contact page, and do not make people scroll past three hero images to find it.
